Terms and Conditions

I understand certain “activities” that my dog may participate in, including daycare, grooming, training, boarding, one-on-one play, movement within and outside the facility and transportation to and from Paw Palace involves risk and possible injury, including but not limited to:

1. Exposure to parasites, viruses, and other medical conditions passed from dog-to-dog or person-to-dog;
• Sprains, strains, bites, broken bones
• Motor vehicle accident during transportation; and
• Fatigue, dehydration, nicks, cuts, extreme stress or death.

2. I further understand that not each and every potential risk can be listed above but, nonetheless agree that the benefits associated with dog socialization outweigh the possible risks, therefore, I hereby voluntarily release, forever discharge, and agree to hold harmless and indemnify Paw Palace and its agents, successors, heirs, from any and all liability, claims, demands, actions, or rights of action, which are related to, arise out of, or are in any way connected with my dog’s participation in activities at Paw Palace, including those allegedly attributable to the negligent acts or omissions of Paw Palace or their staff.

3. Further I understand that I may be exposed to certain risks when bringing my dog to participate in activities at Paw Palace or when picking up my dog from participating in activities at Paw Palace. Such risks may include property damage and/or physical injury inside or outside the facility such as falling, slipping, illness and/or dog bites. Therefore, I hereby voluntarily release, forever discharge and agree to hold harmless and indemnify Paw Palace, its agents, successors, heirs from any and all liability, claims, demands, actions, or rights of action which are related to arise out of, or are in any way connected with my dog’s participation in activities at Paw Palace including those allegedly attributable to the negligent acts or omissions of Paw Palace or their staff.

4. Authorization of Medical Care
If my dog is ill or injured while participating in activates at Paw Palace, Paw Palace will make every reasonable effort to reach me pursuant to the contact information I have provided Paw Palace. However, if Paw Palace is unable to reach me, I consent to Paw Palace seeking appropriate veterinary care and I accept responsibility for any and all associated expenses. Paw Palace will not pay any portion of veterinary expenses associated with seeking medical care for my dog(s).

5. Allergies, Special Diets, Medications
I agree that I will disclose to Paw Palace any allergies my dog may have. I further agree to disclose to Paw Palace any special dietary needs or medication my dog may require if necessary during activities at Paw Palace.

6. Photographs and statements
I authorize use of my dog’s visual image(s) and statements in newsletters, posters, and other materials.

7. Vicious Tendencies
I affirm that I am not aware of any vicious tendencies by my Dog.

8. Agreement to Pay
A. Paw Palace accepts check, cash, or credit cards. I agree to pay the service rates in effect for my dog’s participation in activities at Paw Palace. I further agree to pay for any additional services requested such as grooming, drop off, and or pick up service. A valid credit card must be kept on file at all times. Charges not paid in advance will be charged to Client’s credit card. All services must be paid in full before dog will be released to client. Dogs left 10 days beyond the agreed upon pick up date become property of Paw Palace.

B. Owner understands and agrees that any extended stay longer than 21 days, will require ½ payment up front as well as payments every 21 days until dog(s) are picked up. Your credit card on file will be charged accordingly.

9. Daycare:
• There are no refunds for partial daycare
• Daycare is available Monday – Friday 7:00am – 7:00pm, Saturday 7:00am-3:00pm
• Walk in daycare may not be available during holidays.

10. Boarding:
Paw Palace reserves the right to immediately change your dog’s type of boarding/daycare if we believe it is necessary to protect the health and well-being of your dog, other dogs, or our staff.

Collars/Leashes
For the safety of everyone, all dogs must enter the building on a leash. Dogs are welcome to come in with a prong/chain collar and/or harness/gentle lead, but these will be removed while in day care or boarding. All dogs must wear a nylon or leather flat collar with the name of the dog/owner and owner’s phone number on it. If the dog’s collar does not have this information on it, a Paw Palace identified nylon martingale collar will be provided for their safety.

We recommend you bring your dog’s regular food. We can provide our house food (Nutrisource C+R) for an additional fee, but it is best for your pet to stay on their regular diet. We provide bedding and bowls. You are also welcome to bring your own bedding. You can bring any toys, treats, blankets that you think your dog would like to have with them. Please keep in mind that our guests are not supervised in their suites/kennels 24 hours a day, therefore, anything that may pose as a choking hazard should be avoided. If your dog is destructive or chews things up, we ask that you bring his/her own bed, otherwise you may be charged if the bed we provide is destroyed.

11. Damage
I accept the responsibility of paying for any damage to facility, property, and/or equipment caused by my dog.

12. Veterinary Records:
My dog’s complete veterinary records must be furnished to Paw Palace. These records must include proof of vaccinations and/or treatment for: parvovirus, distemper, Bordatella, heartworm, fleas, and ticks. Records may be sent via facsimile to: 708-757-6119 or email at info@pawpalace.com. I further attest that my dog is free of parasites and other illnesses that can be transmitted from dog-to-dog. Due to the high risk of do-to-dog transmission of such parasites and or viruses I agree that I will immediately notify Paw Palace if I learn or suspect my dog has parasite or viruses and agree to not bring my dog to Paw Palace for any activities until I receive clearance from Paw Palace, in conjunction with my dog’s veterinarian.

13. Evaluation of Dog Prior to Participation
Every dog must be evaluated by Paw Palace prior to participating in any activity. Such evaluation may assess the dog’s temperament and interactions with other dogs and Paw Palace staff.

14. Right to Decline:
I understand that Paw Palace reserves the exclusive right to decline participation or to terminate participation in activities at Paw Palace to any dog at any time for any reason

15. Attorney Fees, Applicable Law and Venue
• Should Paw Palace or anyone acting on their behalf, be required for any reason to incur attorney fees or costs to enforce or defend this agreement, I agree to indemnify and reimburse Paw Palace for such fees and costs. Further I agree and understand that any disputes arising out of this agreement will be decided pursuant to the laws of the State of Illinois and venue shall be in Cook County.

• By signing this document you agree to take no action which is intended, or would reasonably be expected, to harm Paw Palace or its reputation or which would reasonably be expected to lead to unwanted or unfavorable publicity to Paw Palace or its employees.

16. Valid dates
These agreements, waivers and authorizations will remain valid and in force as long as and whenever my dog participates in any activity at or with Paw Palace and for a period of ten years after my dog(s)’ participation.

By signing this document, I acknowledge that if my dog(s) or I am hurt or property is damaged during my dog’s participation activities at Paw Palace, I may be found,, by a court of law, to have waived my right to maintain a lawsuit against Paw Palace on the basis of any claim from which I have released them herein. I have had sufficient opportunity to read and fully understand this entire document and I agree to be legally bound by its terms

We accept Visa, MasterCard, Discover, cash or check (please note, there is a $25 returned check fee). Final payment is due upon pickup of your dog. Paw Palace reserves the right, without notice, to adjust its fees for services. Please inquire at the front desk or on our company website as to our current fees.
